Social Prescriber Services available as SELF-REFERRAL SERVICE

Great news as the Social Prescribing self-referral is open to all PCN2 Practices and now allows patients registered at East Finchley Medical Practice to self-refer on to the service.

Please note that only the following referral reasons are available to select: Addiction Support, Bereavement Support, Carers Support, Digital Inclusion, Emotional Wellbeing, Employment support, Financial Advice, Loneliness / Isolation. Anyone outside of these reasons of referral, or perhaps more complex in need, would have to be referred via the Practice.

The practice complies with Data Protection and Access to Medical Records legislation.  Identifiable information about you will be shared with others in the following circumstances:

  • To provide further medical treatment for you e.g. from district nurses and hospital services.
  • To help you get other services from the social work department.  This requires your consent.
  • When we have a duty to others e.g. in child protection cases.  Anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

All patient information is considered to be confidential and we comply fully with the Data Protection Act.  All employees have access to this information in relation to their role and have signed a confidentiality agreement.  Information may be shared, in confidence, with other NHS organisations in the interest of patient care.  Reception and administration staff require access to your medical records in order to do their jobs.  These members of staff are bound by the same rules of confidentiality as the medical staff.

Your personal details and medical records are kept in confidence. Information stored on computer is governed by the Data Protection Act. To maintain confidentiality, you will be asked certain personal details before giving your test results over the telephone.

Please note that it is the Practice's policy to record all telephone calls for the purposes of patient and staff care, security, and dispute resolution.  Recordings and their use will be at the partner's discretion and will also comply with the Practice's Data Protection registration.

Access to your medical records

You can apply for online access to your medical record, along with requesting medications and booking appointments through Patient Access or the NHS APP. Please complete the application form:

Alternatively, if you wish to receive copies of your medical records, you can apply for a subject access request. If you wish to receive copies of your medical records, please complete a Subject Access Request APPLICATION FORM and return it to the practice. Requests will be dealt with within one month.

Should your doctor decide that you seeing your records might put your health at risk, you may only be shown part of the records.
