Social Prescriber Services available as SELF-REFERRAL SERVICE

Great news as the Social Prescribing self-referral is open to all PCN2 Practices and now allows patients registered at East Finchley Medical Practice to self-refer on to the service.

Please note that only the following referral reasons are available to select: Addiction Support, Bereavement Support, Carers Support, Digital Inclusion, Emotional Wellbeing, Employment support, Financial Advice, Loneliness / Isolation. Anyone outside of these reasons of referral, or perhaps more complex in need, would have to be referred via the Practice.

NHS REFERRALS:

If your GP has referred you for further health advice or treatment, NHS Barnet Referral Management Service will assess or process your referral.
If you have not heard about your referral within 4-6 weeks, please contact:

PRIVATE REFERRALS:
Please note that any private referrals can take up to one week to be typed before they are ready for collection.  You can arrange an appointment to see your private consultant before you have received your referral letter.
Conversion of NHS to Private Referrals:
To convert NHS referrals to Private Referrals, please contact reception who will inform the secretaries.  Please note:-
1.  The private letter will take up to 3 days to generate and you will need to collect it from reception.
2. For insurance purposes, you can still see the consultant without the referral letter and be covered by your insurance company as long as you forward the letter once generated to the consultant.
